Genetic Risk
ADPKD is an autosomal dominant genetic disorder. If one parent is affected, each child has approximately a 50% chance of inheriting the disease-causing gene. About 10% of patients have de novo mutations with no clear family history.
- Inheritance pattern: Autosomal dominant; males and females are affected equally
- Child risk: Each pregnancy is calculated independently, with approximately a 50% probability
- De novo mutations: About 10% of patients have no family history, caused by new gene mutations
Genetic Testing
Genetic testing helps confirm the diagnosis, guides risk assessment for family members, and informs reproductive decisions. It is recommended to undergo testing through a genetic counseling department, with results interpreted by a professional.
- Major genes: PKD1 (approximately 85% of cases), PKD2 (approximately 15%)
- Timing: Recommended before planning pregnancy to allow sufficient time for decision-making
- Where to go: Visit a hospital genetic counseling department or prenatal diagnosis center
- Significance of results: Once the disease-causing mutation is identified, it provides the basis for prenatal diagnosis or embryo screening
Note
Interpreting genetic test results is complex. Some variants are of uncertain significance (VUS) and require a genetic counselor to evaluate in the context of family history β do not draw conclusions on your own based solely on the report.
Prenatal Diagnosis
When the family's disease-causing mutation is known, prenatal diagnosis can determine whether the fetus carries that mutation. The two common sampling methods and their timing windows are as follows.
- Chorionic villus sampling (CVS): Usually performed at 11β13 weeks of pregnancy, sampling placental chorionic tissue for testing
- Amniocentesis: Usually performed at 16β20 weeks of pregnancy, sampling amniotic fluid cells for testing
- Prerequisite: The disease-causing mutation site must first be identified in the family
IVF-PGT (In Vitro Fertilization + Preimplantation Genetic Testing)
For families who wish to avoid passing the disease-causing gene to their children, IVF combined with preimplantation genetic testing (PGT) can be used to select embryos that do not carry the mutation for transfer.
- Principle: After in vitro fertilization, embryos undergo genetic testing, and embryos without the disease-causing mutation are selected for transfer
- Applicable to: Families with a known family disease-causing mutation who meet assisted reproduction criteria
- Limitations: The process is lengthy, costs are high, and success rates are influenced by multiple factors; assessment by a reproductive medicine center is required
Pregnancy in Women with ADPKD
Most women with ADPKD who have normal kidney function can safely become pregnant, but a joint pre-pregnancy assessment by a nephrologist and obstetrician is needed.
- Normal kidney function: Pregnancy is usually safe, with enhanced monitoring during pregnancy
- eGFR < 40: Risk assessment is needed; reduced kidney function increases the risk of pregnancy complications
- Tolvaptan: Contraindicated during pregnancy; must be discontinued under medical guidance before planning pregnancy, with a washout period
- Pregnancy monitoring: Blood pressure, kidney function, and urine protein require close follow-up

Some medications, such as tolvaptan, pose potential risks to the fetus. Pre-pregnancy medication adjustments must be guided by a doctor β do not stop medications on your own. If you experience severe headache, blurred vision, or reduced urine output during pregnancy, seek immediate medical care or call your local emergency number.
Contraception Choices
When choosing a contraceptive method, women with ADPKD need to consider their liver cyst status.
- Estrogen-containing contraceptives: May accelerate liver cyst growth and are generally not recommended for long-term use
- Progestin-only methods: Such as progestin-only oral contraceptives or subdermal implants, can be used as alternatives
- Non-hormonal methods: Such as condoms or copper intrauterine devices (IUDs), do not affect hormone levels
- Individual choice: The specific method should be discussed with a gynecologist
Male Patients
ADPKD usually does not affect male fertility. If severe hypertension or declining kidney function is present, it may indirectly affect overall health; a pre-pregnancy health assessment is recommended.
Important Emphasis
